Human Anatomical Materials Review Committee (HAMRC) Forms

The USU Form 3213, HAMRC Application, needs to be completed and submitted before the application can be reviewed by the Human Anatomical Materials Review Committee (HAMRC).  HAMRC forms are now submitted online via Google Forms. If you are submitting a HAMRC application to use anatomical materials in a Research project, a copy of your protocol and Form 3208 should be submitted as well.  Please be as specific as possible regarding the procedures planned on the materials, as this will ensure a more efficient review of your application.  Please submit the HAMRC package at least 8 weeks prior to the date you wish to start your activity for all new activities; four (4) weeks in advance for repeat/annual activities.  Prior to submission, you should contact Mr. Ron Rivenburgh in the ATL to ensure that he will have the ATL space and anatomic materials on the dates you request.  Failure to do this step may result in your application review being delayed.  Upon completion of your activity, please submit a USU Form 3215, HAMRC Final Report, also available electronically as a Google Form.  If you will be requesting to produce imagery as part of your activity, please read the HAMRC Imagery Policy and complete the applicable section in the Form 3213.
