USU Commissioning

Physical Qualifications

Notification of your physical status is issued through regular postal mail directly from DoDMERB approximately 4 to 6 weeks after the submission of your examination results.  You may schedule your physical online.  You may also check your medical status.

In addition to being medically qualified by DoDMERB, you are also required to meet accession standards for height and weight.  Even if you are notified by DoDMERB that you are “Qualified,” if you are not within the required weight standards you will need to provide documentation of being within body fat composition standards (DA Form 5500 (Male) or DA Form 5501 (Female)).  You will be contacted via email if you are not within standards.

Remedial Requested

If you have “Remedials Requested”, this means that after reviewing the results of your examination, there may be additional tests, evaluations, or information required to make a qualification decision.  The requirements are listed in your notification letter, as well as guidance for completing the remedial(s).

Medically Disqualified

If you have been determined to be “Medically Disqualified”, this means that after reviewing the results of your examination, a condition(s) that does not meet the medical accession standard has been found.  You should contact the Commissioning Coordinator immediately to discuss your options.  Please fill out the Waiver Request Form.

Waiver Process

If you initiate waiver action, there may be additional medical tests, evaluations, and/or consultations that must be obtained and evaluated prior to determining your final medical status.  Please keep in mind that additional information may lead to further inquiries or requirements.

Be prompt in satisfying any requests for additional information, tests, evaluations, and/or consultations.  It is imperative for your application that these issues be addressed or resolved as soon as possible.

The waiver process can take 6 to 8 weeks.  This is an average time frame and may be longer or shorter depending on your individual situation.
