Submission Requirements for Clinical Instructor and Assistant Professor

General Faculty CAPT guidelines for academic appointments.

For individuals interested in applying for the position of Clinical Instructor or Assistant Professor within the Department of Rehabilitation Medicine, please submit the following documents:

  • Curriculum vitae (CV) with bibliography. (Please use format of Dr. Rice's example)
  • Contact Information
  • Complete sections 1,2,3,10,11, and 12 of  USUHS Form 107
  • Letter of Intention – How will you contribute to the Department of Rehabilitation, Uniformed Services University and/or Military Healthcare System
  • Letter of recommendation from Supervisor/Commander Officer. The letter must be on official organization letterhead and highlight the applicants achievements and qualifications

Submission Requirements for Rank of Associate Professor and Professor

General Faculty CAPT guidelines for academic appointments.

For individuals interested in applying for the position of Associate Professor or Professor within the Department of Rehabilitation Medicine, please submit the following documents:

  1. Complete USUHS Form 107.
  2. Curriculum vitae (CV) with bibliography. (Please use format of Dr. Rice's example)
  3. Abbreviated CV [Please use format of Board of Regents (BOR) abbreviated CV]
  4. A personal letter addressed to the Committee of Academic Promotions and Tenure (CAPT) at the Uniformed Services University, which highlights your unique expertise and significant accomplishments in terms of research, clinical instruction, leadership, and/or impact that you have had on your specialty locally, regionally, nationally, or internationally.
  5. A minimum of four (4) External Letters of Evaluation for Associate and six (6) for Professor - to be obtained from the USUHS sponsoring department. Please provide a list of potential External Reviewers and their contact information with your application. (Note: All external letters must be signed and printed on letterhead).
  6. A minimum of one (1) letter of USU peer evaluation, by individuals of a higher academic ran
  7. Memoranda from chief/director/or commanding officer of the organization where the candidate is principle assigned.
  8. Samples of Publication Materials - Maximum of four reprints.
