SOM Class 2018 Registrar Documents Request

We hope to facilitate your journey to graduation from Uniformed Services University (USU) through our student-centered philosophy. We will provide the highest level of customer service, professionalism, courtesy, and efficiency in responding to your requests.

The Registrar’s Office is the center of the registration process at USU, investigating and finalizing details as to when and how registration will proceed, compilation of students' schedules, and assisting students in all aspects of registration. Students typically are registered by their degree program office or select courses in  Empower.

The USU University Registrar functions as a service office to provide students, alumni, faculty, administrators, military and other institutions with the necessary support as we maintain each student's academic record with integrity, respect and confidentiality. We emphasize the importance of treating all members of our community with respect and fairness. We comply with all federal, state and University rules and policies with regard to privacy. 


To request Records

  • For official and unofficial transcripts, matriculation/graduation letter of dates and degree received, Dean's Letter/MSPE (for Doctor of Medicine) complete this form:
  • For Verification of Diploma, Licensure Form, Deferment Form, and any other accompanying document, send the document as an email attachment to:
  • For third party degree verification e.g, employment, background screenings, or other agency requesting this information please submit your request via email along with a signed authorization by the individual to release.


Verification of Diploma issued before 1990

If you require a verification of diploma prior to 1990, please scan and email a legible copy of the Diploma to  Normal processing time is 5-7 business days.

Replacement Diploma

In the event your diploma is lost, stolen, damaged, or you legally change your name, a request for a replacement diploma may be sent using our Diploma Request Form.  The cost for a replacement diploma is $21.18 made out to Oak Hall Cap & Gown.  Normal processing time is approximately 3 months.  The new diploma will bear the current names of the officials in office at the time the replacement is produced.  We do require, if available, original diploma be sent to us prior to process your request.  Your diploma request form, check, and original diploma should be mailed.  For additional information contact us at
